Please use these downloadable PDF forms for signing up at Buff Buddies. Print out, fill out and bring it in. "Click" on the blue below to download the forms you need.

• Buff Buddies Fitness Sign-Up Forms: Use this form if you're signing up your child or children - the first line is for the childs name (you can add all your children on this line, just seperate them with a slash (/). The third line is for their birthday including the year (please use BIRTH year), you can also seperate them with a slash (/).

• Party Release Form: This form is used when you're having a party (birthday, graduation, family get together, etc.) The first page is filled out by the family organizing the party and bring in the second page for those who are attending to fill out.

• Check Authorization Form: Print out this form and follow the instructions, then bring it in when you enroll your child/children.

• Credit Card Authorization Form: If you don't want to use a check. Print out this form and follow the instructions, then bring it in when you enroll your child/children.

• Bank Authorization Form: Print out this form and follow the instructions, then bring it in when you enroll your child/children.
